Prior to your visit
We respect your time and schedule. As a result we offer several online features to make your arrival smooth and wait time to be at a minimum. We have created several online options to better facilitate your visit.
OPTION 1: You can use the Secure Online PATIENT INTAKE FORM to complete both the Patient Registration process and to indicate your areas of pain & discomfort. As a First-Time patient, please fill the form completely; including your insurance information. On subsequent visits you need only to complete the Patient Information and Symptoms section.
We encourage you to use the online form prior to your visit to the office. There is nothing to print. Once you have completed the Online Form and clicked on the word SUBMIT the information will automatically be quickly and securely be imported into our system. We'll be ready for your arrival well-prepared.
OPTION 2: You may scroll down to the bottom of the page and click the PRINT FORM option to print a hard copy of the form and hand carry to your visit.
OPTION 3: On-Location - Please arrive 20 minutes prior to your scheduled appointment time to use our Patient Kiosk to complete the Welcome Process
Once you've been registered as a new patient, you will be given a personally unique secure code to use for future visits when using the Patient Kiosk to check-in.
When entering our large and comfortable lobby area, please introduce yourself to one our friendly receptionists. You will be well-greeted as a welcomed site. If you need any assistance with the Kiosk, please let one of our staff members know and we will be more than happy to assist you. This process is important as it will provide us with general information about you, the purpose of today's visit, your current condition, and history of care.
For your security, the Staff will scan your Government-Issued Photo ID which will then be imported into your Electronic Patient Health Record located on our Encrypted & Secured Server.
Next you will have a consultation with the doctor to discuss your health-related problems, concerns, as well as potential treatment options. Your first visit is designed for the doctor to learn more about you, your condition and your expectations to determine whether chiropractic care meet your goals.
After your consultation, the doctor will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neurological, orthopedic, postural, and physical examinations. Nothing will be done in our office without your consent.
Your specific condition may require us to order x-rays so as to perform a more thorough evaluation. X-rays help us rule out more serious conditions or assist us in developing the most effective treatment plan for you. Most people are amazed once they see their x-rays and can often immediately identify their misalignments and degeneration themselves. You will be referred to a third-party facility who specializes in taking the safest and most accurate approach to minimize unnecessary exposure to X-rays. If you are being referred to us by a medical doctor and have had recent X-ray workups, please let both our Front Desk staff and your doctor know so that we may request these films from your previous doctor. Again, we care about your safety and do not wish to subject you to unnecessary exposure.
Report of Findings
Once all the information and examinations have been performed, the doctor will give you a report of findings to answer the four most frequently asked questions:
- What is the source of my pain?
- Can a chiropractic treatment plan help me?
- Will I need to be referred out to a Medical Doctor?
- How much is it going to cost?
After discussing your history and your goals with you, examining your spine and reviewing any x-rays or tests, the doctor will discuss the most fitting course of care. If you have a condition that requires care with other providers, the doctor will tell you this. It's at this point that you should feel free to ask any questions and discuss any concerns.
Patients will generally be provided with their first treatment at the end of their first consultation. This may include spinal adjustments, physical therapies, and/or soft tissue therapies. Treatment is interactive and, again, you are encouraged to express any question you have about the applicability of the particular step in the treatment.
Prior to leaving, patients will be instructed on a wellness program to be conducted at home. If you are in pain when you first come into our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. If you desire an overall wellness lifestyle, our wellness team will work with you to create good habits and routines that support your success. Every body is unique and different and therefore everyone requires a unique and customized wellness plan.
